Better Balance for Life – A fall prevention program incorporating balance, strength and home safety. Our team of professionals works with your physician to create a customized treatment plan to improve mobility and prevent falls and injuries.
Total Joint – A program incorporating all aspects of a Joint Replacement: Pre-surgery, home health rehabilitation, and outpatient physical therapy. Our goal is to provide a seamless transition to regaining independence.
Lymphedema Therapy – Lymphedema is swelling (edema) that occurs in the legs, arms or anywhere in the body. Central Coast Home Health has certified lymphedema therapists who can safely and effectively treat lymphedema and teach self-management to minimize re-occurrence. Our goal is to help patients and families live life to the fullest.
Congestive Heart Failure – For those battling Congestive Heart Failure, this four-week program offers a comprehensive approach of a Registered Nurse, Physical Therapist, and Registered Dietitian to help patients achieve the best quality of life possible. Our program is entirely individualized and excels in providing patient and family education, a tailored activity plan and lifestyle modification.
Parkinson’s Wellness – Traditional outpatient physical therapy is not a conducive environment for making lasting changes in Parkinson’s patients. Central Coast Home Health’s Parkinson’s Wellness Program is a specific evidence-based protocol and individualized exercise program aimed at any stage of Parkinson’s. Our goal is to delay the symptoms and keep quality of life at its best.
Palliative Care – “Palliate” means to ease. Our program is focused on providing patients with relief from the symptoms, pain, and emotional distress of a serious illness such as cancer, heart disease, lung disease or kidney failure, among others. The goal of the program is to improve the quality of life for both the patient and the family.
Skin and Wound Management – This specialized area focuses on overall skin care and promotion of an optimal wound environment, including prevention, therapeutic interventions, and rehabilitative interventions. We now provide the community with a growing number of Wound Care certified clinicians.
